Our 5 Step Process to Participate in Produce Prescription Program with FEI
STEP ONE
Review our website and schedule an introductory meeting to learn how our program services can improve patient health outcomes.
STEP TWO
Sign MOU and/or BAA and Service Agreement documents for de-identified data sharing to determine the effects and outcomes of the program services.
STEP THREE
Share awareness of program with staff and educate how to screen and refer patients for produce prescription program.
STEP FOUR
Send FEI referrals and visit with program participants at least every 6 months.
STEP FIVE
Share de-identified data for research purposes and program assessment and feedback.
Food is Medicine Produce Prescription Program
Home delivery of nourishing food as medicine for patients experiencing food insecurity and diagnosed with prediabetes or Type 2 Diabetes.

Service
Produce deliveries will be a step down model initially with 4x/mo home deliveries. -

EDUCATION
All patients have access to culinary and nutrition education via the Food Equality Initiative website.
